Open Question: How do I find the proper legal format for an answer to a court summons in Oregon?

I recently received a summons from a debt collector and need to file my "answer/motion" within 30 days. I fully believe I can fight this without a lawyer, however, I am getting hung up when trying to find the proper legal format for my answer.
